Starting Salary: $39,052 - $48,579

 (* Based on 2080 hours per year)

  

SUMMARY OF JOB RESPONSIBILITIES:

  • Work with all Activity Staff in order to create and implement unique programs within the Six Dimensions of wellness for the Residents in Crown Center and Greenwood.
  • Evaluates the effectiveness of staff, programs, and residents’ individualized goals
  • Completes required documentation (MDS, initial history, progress notes progress notes)
  • Active member on the Plan of Care team
  • Works with volunteers within the facility (responsible for initial interview of community adult and student volunteers and appropriate placement)
  • Works with Activity Assistants in creation of monthly calendar for Crown Center and Greenwood
  • Responsible for creation of monthly calendar
  • Responsible for scheduling of activity/programming staff
  • Implements programs within Crown Center and Greenwood as needed
  • Works with Programming/Activity Manager in maintaining budget for Crown Center, Greenwood.
  • Maintain Activity Policy and Procedure Manual
  • Complete daily documentation on all residents
  • Complete assigned one to one sessions
  • Daily tasks as needed (aviary care, daily happenings, room set ups, etc.)
  • Maintain office supplies and order supplies/equipment as needed
  • Schedule rooms as needed
  • Performance of other duties as deemed necessary and appropriate

 

 

QUALIFICATIONS

 

EDUCATION:

  • BS degree in Therapeutic Recreation or related area
  • Would prefer individual is a Certified Recreational Therapist Specialist

 

EXPERIENCE:

  • Would prefer individual with minimally 2 years experience as Activity Director in a Long Term Setting
  • Minimally 5 years  Activity experience in Long Term Care or Assisted Living setting

 

KNOWLEDGE AND SKILLS:

  • Ability to write individualized outcome specific goals
  • Ability to provide adaptations for successful engagement for residents with physical and cognitive challenges
  • Good communication skills, must be able to demonstrate effective reading, writing, and oral skills
  • Must be detailed oriented person and be able to perform duties with great accuracy
  • Competent in Microsoft Word, Excel, Outlook, Internet
  • Must be able to interface well with others in a positive and professional manner
  • Ability to work in a group setting or individually
  • Knowledge of disease processes and behaviors of the elderly
  • Sensitivity to older adults
  • Knowledge on proper body mechanics
  • Knowledge of proper techniques for transferring residents
  • First Aid
  • CPR

 

PRINCIPAL JOB ACCOUNTABILITIES:

  • Meets productivity standards, completes work in a timely manner, (ie: implementation of programs on time)
  • Completes required documentation in all areas
  • Manages Activity Staff
  • Maintains daily activity attendance records
  • Plans, implements, and evaluates programs in all areas of care
  • Responsible for interviewing and placement of community and student volunteers in Crown Center, Greenwood using them effectively and efficiently
  • Completes one to one visits with those residents needing individualized attention
  • Communication to team members/interdisciplinary team necessary information of residents and/or programs
  • Maintains budget
  • Working with team members in completing variety of tasks required on a daily basis (aviaries, happenings, daily boards, maintaining office/lounge spaces)

 

JOB ACCOUNTABILITIES – Other:

CONTINUING EDUCATION REQUIREMENTS:

  • Annually 4 hours CEU related to dementia
  • 50 hours of CEU in a five year period in order to maintain certification in Therapeutic Recreation
  • CPR
  • First Aid
